I would like to thank you for
choosing New Horizons for your training needs. Your continued support of our center
is sincerely
appreciated and extremely important to us; that is why we are asking you to take a few minutes
to evaluate our performance.
Our evaluation system uses a scale of 1 to 9. Please take note that, on this scale, 1 is the lowest score and 9 is
the highest score.
At New Horizons we strive to provide
an outstanding experience every time. If for any reason we did not meet your expectations,
please take a moment to write comments so that we know how to improve your next
experience. We also like to know those areas where we exceeded your expectations.
If something stands out, please note that as well.
In addition, if your employer sponsored
your attendance please remember to fill in the 'Company' field on the evaluation.
Many organizations request that we
provide feedback to them as part of our training services. We can only do that if
the company name has been entered on the evaluation.
